ALA is proud to announce that we have expanded our case management services to include healthcare navigation. We welcome our new ECM team, Program Director Jaime Carrillo, Care Managers Janine Stewart, Erica Gutierrez, Arelly Otelo, and Community Health Worker Tanya Garcia.
A New Way to Care for Your Health
ECM is a new, statewide Medi-Cal reform that includes social services as a covered benefit. ECM provides a care manager to support and assist individuals suffering from complex or chronic healthcare needs. If you have a complex health condition that requires more than just medical treatment and struggle with homelessness, mental illness, substance abuse, or frequent hospital visits, or just need extra support and guidance to manage your health and well-being, ECM can help.
ECM Services
When you enroll in ECM, you will be assigned a dedicated case manager who will work with you to create a personalized care plan. Your case manager will also coordinate with your primary care provider, specialists, behavioral health providers, and other community partners to ensure that you receive the best possible care.
Your case manager will provide you with:
- Healthcare Navigation: Your care manager will monitor your health status, follow up with your appointments, help you manage your medications, and assist you with any issues that may arise.
- Advocacy: Your care manager will advocate for your rights and preferences, help you navigate the healthcare system, and connect you with the appropriate services.
- Health coaching: Your care manager will educate you about your health condition, teach you self-care skills, and support you as you work to achieve your health goals.
- Referrals to community resources: Your care manager will refer you to resources and services that can help you with your non-clinical needs, such as housing, food, transportation, legal assistance, and more
